To promote successful living by improving the quality of healthcare services from Adolescence through Geriatrics with prevention, evidence-based medicine, and an interdisciplinary team approach using the Medical Home Model of care.  

Dear Harpeth Family Medicine patrons,

In this time of crisis, we wanted to reach out to you to announce our new Telehealth service.  With social distancing due to the pandemic, Harpeth Family Medicine is providing Telehealth to all of our patrons.  This begins immediately with all of your upcoming appointments, any acute concerns you have, and even new patients appointments.  

Our office administrator will be reaching out to you as your scheduled appointment becomes closer to give you instructions on obtaining labs and how the process of the visit will work.

If you have any acute symptoms or concerns, please don’t hesitate to reach out to us and let us know how you are doing so we can provide the best management for your problem.  We can set up Tele-visits as soon as you need to assess and treat your problem.  

Our practice is also changing to providing primary care at home.  In the near future, our team will be coming to see you to provide primary care in the comfort of your home.  We call it Harpeth Family Medicine @Home.  Instead of coming to see us in the office, we will come to see you for your acute care as well as chronic disease management and yearly preventative health.  With this new model we will be able to stay better connected to you while providing the best care possible.  

Everything else stays the same with Harpeth Family Medicine in this new model - same great service and prompt response, along with coordination of care from our office staff, Tamra and Brooke.  We also have two midlevel providers, Tiffany and April, who are working with our team to provide care.  

Until this pandemic is controlled for everyone’s safety, all visits will be changed to Telehealth, including all your acute problems.  Our new model of care providing primary care @home will be started once we are clear of this current crisis.
